What Clients See When Completing Intake Forms

QuickerNotes automatically generates intake forms based on your form templates and send rules. When a form is created, the client receives an email with a secure link to complete it online.


How to Preview What Clients See

You can view the exact version of the intake form your client will see:

  1. Open the client’s profile.

  2. Go to the Forms tab.

  3. At the top of the page, click Open Client Forms.

  4. Choose whether to:

    • Stay logged in, or

    • Log out to view the full client-facing version

  5. You’ll be taken directly to the client’s pending form(s).

This is the best way to verify client experience before an appointment.



What Clients Receive

Clients receive:

  • An automated intake form email (based on your Send Rules)

  • A secure link to their pending form

  • Your clinic’s name

  • The reply-to email you configured

  • Your customized email messaging



Client Experience Overview

Clients will see:

  • A mobile-friendly intake form

  • Required questions clearly marked

  • Policy acknowledgement and/or signatures when configured

  • Ability to switch languages at the top of the page

Once submitted, the form appears instantly in the client’s profile in QuickerNotes.
