SOAP notes are created directly from client appointments. You can write a free-form Text Note or use a structured SOAP Form Template. Providers will only see the templates that have been assigned to them.
Create a SOAP Note From an Appointment
Open the client’s appointment page.
Click New Note.
Choose one of the following:
Text Note — always available
SOAP Form — only templates assigned to you will appear
This is the primary way of creating SOAP notes.
Create a SOAP Note From the Client Profile
Open the client’s profile.
Go to the Appointments tab.
Select the appointment.
Click New Note → choose Text Note or an assigned SOAP Form.
Why You May Not See a SOAP Form Template
Providers see only the templates assigned to them.
Templates are assigned through:
Settings → SOAP Form Templates → Assignees
If a template does not appear, it may be:
Not assigned to your user
Missing required permissions
Newly created but not yet assigned
Inactive or unassigned after updates
Create an Unlinked Note (Text Notes Only)
To document without an appointment:
Open the client’s profile.
Go to the Unlinked Notes tab.
Click New Unlinked Note.
Unlinked Notes can only be Text Notes.
SOAP Forms require an appointment.

How Date of Service Is Set
Appointment notes use the date of the linked appointment.
Class notes use the date of the linked class visit.
New unlinked Text Notes start with the date the note was created.
Notes copied from History use the selected visit's date. If you do not select a visit, the copy is created as an unlinked note and does not automatically match a nearby appointment.
Linking or relinking a note changes its Date of Service to the selected visit's date. The date can be edited directly only on an unlinked Draft written by you, when your role has permission to create notes.
