Reset SOAP Notes Back to Draft Status

You can reset a Signed & Completed SOAP note back to Draft status. This unlocks the note for editing, removes all cosigners, and notifies them that the note has been reset.


Reset a Note to Draft

  1. Go to SOAP Notes, on the left-side sidebar.

  2. Locate the Signed & Completed note you want to reset.

  3. Click Reset Draft.

  4. The note returns to Draft status and becomes editable again.

When a note is reset:

  • All cosigners are removed

  • All cosigners receive a notification

  • You can edit/delete the note as needed


Addendums After Reset

Resetting a note to Draft also allows you to delete addendums that were previously locked.

Permissions Required

To reset a note to Draft, your role must include the reset-draft permission located under:

Settings → Roles & Permissions → Notes → reset-draft

Reset to Draft and Date of Service

Resetting a completed note to Draft does not change its Date of Service.

  • If the note is linked to an appointment or class, its date continues to come from that visit and there is no separate Date of Service editor.

  • If the note is unlinked, the original author can edit the date after the reset only when their role has permission to create notes.
