Common Questions & Limitations of Clara AI

Clara AI is designed to assist with clinical documentation, but like any AI-powered tool, it has limitations. Understanding when Clara works best — and when it should not be used — helps clinics get the most value from the feature.

This article addresses common questions and sets clear expectations.

How Accurate Is Clara AI?

Clara AI is generally effective at transcribing and structuring clinical conversations, but accuracy can vary based on several factors, including:

  • Audio quality

  • Background noise

  • Speaking clarity

  • Multiple people talking at once

Providers should always review and edit AI-generated notes before saving them to the chart.

Clara is a documentation aid, not a substitute for provider review.

Does Clara AI Work in Noisy Environments?

Clara performs best in quiet or moderately controlled environments.

Accuracy may be reduced in situations such as:

  • Loud treatment rooms

  • Multiple conversations happening simultaneously

  • Background music at high volume

For best results, clinics should minimize background noise when possible.

Can Clara AI Be Used for Group Visits or Multiple Speakers?

Clara AI is optimized for one-on-one provider–patient visits.

Limitations may occur when:

  • Multiple providers speak during a visit

  • Group appointments are conducted

  • Several people speak over one another

In these cases, documentation may require more manual editing.

Should Clara AI Be Used for Every Visit?

No. Clara AI is optional and should be used at the provider’s discretion.

Providers may choose not to use Clara for:

  • Very short visits

  • Highly technical procedures

  • Visits involving sensitive conversations

  • Situations where recording is not appropriate

Manual documentation remains fully supported in QuickerNotes.

Does Clara AI Automatically Save Notes?

No.

Clara AI does not automatically finalize or save notes.

Providers must:

  • Review all AI-generated content

  • Edit as needed

  • Manually save the note to the chart

This ensures providers maintain full control over documentation.

What Happens If the AI Misses Something?

If Clara AI misses details or misinterprets part of a conversation:

  • Providers can manually edit the note

  • Notes can be supplemented just like any other documentation

AI-generated notes should be treated as a draft, not a final record.

Can Patients Decline Being Recorded?

Yes.

If a patient does not consent to recording:

  • The visit should not be recorded

  • Providers can document the visit manually in QuickerNotes

Clara AI use is optional on a per-visit basis.

Is Clara AI Required for All Providers?

No.

Clinics can choose:

  • Which providers use Clara AI

  • Which visits are recorded

  • When AI transcription is enabled

Clara AI does not replace existing documentation methods.

Summary

Clara AI is designed to:

  • Save time on documentation

  • Reduce after-hours charting

  • Support provider workflows

It works best when:

  • Audio quality is good

  • Visits are one-on-one

  • Providers review and edit notes

It should not be treated as:

  • An autonomous clinical system

  • A replacement for provider judgment
