Submit a Recording & Generate a Note

Use this workflow from the Submit Recording page after recording audio, uploading a file, or entering a transcript.


Generate a Note (Step-by-Step)

Select a Patient

Choose the patient you would like to create a note for.

Select a Recording Type

Choose the appropriate recording type:

  • Dictation → Single speaker

  • Conversation → Two speakers


Record

Record the visit, conversation, or dictation.

Add Missing Details

Include any information you would like Clara to incorporate that was not mentioned in the recording.

Click Generate

Continue to the note generation screen.


Choose an Output Format

Select how you would like the recording applied:

  • Text Note

  • Form Note

  • Document

(Optional) Link to a Visit

Associate the generated note with a specific appointment.

(Optional) Apply a Clara Template

If Text Note or Document is selected, choose a Clara template to guide how the note is generated.

(Optional) Add Previous Notes for Context

Select up to two previous notes for Clara to reference during note generation.

Generate Note

Once your settings have been configured, click Generate Note.

Clara will begin processing immediately.


Applying a Recording to a Form Note

There is no note-type selector on the Submit Recording page.

If you want Clara to generate content for a Form Note, you must:

  • Record directly inside the Form Note, or

  • Select the target Form Note after recording (when prompted)

Clara applies output to the note that is actively targeted.​


What Happens After You Click Generate

  • Clara creates or targets a draft note.

  • An AI job is started.

  • You may be redirected to the target note (if applicable).

Job progress appears in:

  • Recordings

You can also view and apply other Clara recordings for the same client, not just the one you just submitted.

Within the note view, you can toggle between:

  • This Note (recordings tied to the current note)

  • This Client (all recordings for that client)

  • All Recent (all recordings made by the provider)

This allows you to review or apply output from prior recordings if needed.

All generated content must be reviewed before finalizing.


Important Limits

  • Only one active AI Scribe recordings per staff user at a time.

  • Transcript submissions must contain meaningful content. Very short or empty transcripts will be rejected.

  • Clara generates draft content only — it does not finalize or sign notes automatically.
