Addendums

Addendums allow you to make additional updates or clarifications to a SOAP note after it has been Signed & Completed. Each addendum is logged with the author’s name and timestamp for full audit tracking.

Any staff member with permission to view and edit notes can add an addendum.


Add an Addendum

  1. Open a Signed & Completed SOAP note.

  2. Scroll to the bottom of the note.

  3. Click Add Addendum.

  4. Enter your update and save.

Your addendum will appear below the main note, labeled with your name and the date it was added.


Delete an Addendum

You can delete addendums only when the note is in Draft status.

How to Delete an Addendum

  1. Reset the note back to Draft (see Reset Draft article).

  2. Once the note is in Draft, a Delete button will appear next to each addendum you are allowed to remove.

  3. Click Delete to remove it.


Who Can Delete Addendums

  • Note creator:
    Can delete any addendum on that note.

  • Addendum creator (who is not the note creator):
    Can delete only the addendums they personally created.


Permissions

To add or manage addendums, users must have the appropriate SOAP note permissions under:
Settings → Roles & Permissions → Notes
