SOAP Form Templates allow you to design structured SOAP forms that providers complete during documentation. Templates are built and managed in Settings and must be assigned to staff before they appear as options when creating a note.
Create a New SOAP Form Template
Go to Settings → SOAP Form Templates.
Click New SOAP Template.
Enter the template name.
Add your custom questions (see below).
Click Save in the lower-right corner.
Once saved, the template becomes available for assignment.
Edit an Existing SOAP Form Template
Go to Settings → SOAP Form Templates.
Existing templates appear below the New SOAP Template button.
Click Edit next to the template you want to modify.
Add, reorder, or remove questions.
Click Save.
Edits apply to all future notes created from that template.
Assign a Template to Staff
Providers only see templates assigned to them when creating a SOAP note.
To assign a template:
Go to Settings → SOAP Form Templates.
Click Assignees on the template you want to assign.
Select the staff who should have access.
Click Save.
When creating a note, providers will only see:
Text Note (always available), and
The SOAP Form Templates assigned to them
Unassigned forms will not appear in the note creation menu.
Custom Questions
Each template can contain multiple custom questions. Supported question types include:
Free Text
Number
Options — Select Many
Options — Select One
Image
Text Area
Add a Question
Open a template and click Edit.
Under Custom Questions, click New Question.
Complete the fields:
Question text
Question type
Help text (optional)
Required toggle
Repeat for all questions.
Click Save.
Remove a Question
Click the arrow next to the question.
Click Remove Question.
Save the template.
Reorder Questions
Use the drag handles along the left side of each question.
Move questions up or down to change their order.
Click Save.
Reordering changes the layout providers see when completing the SOAP Form.
Limitations & Behavior
SOAP Forms cannot be used to create Unlinked Notes.
CPT/ICD insurance codes cannot be added to SOAP Forms—only to Text Notes.
Templates must be assigned to staff before they appear.

After a SOAP Form note is completed:
Addendums may be added
Notes may be Reset to Draft (permissions required), which removes cosigners and unlocks the content
