The Appointment Note Completion report shows whether completed appointments have signed SOAP notes, for each provider, over a date range you choose. Use it to monitor provider compliance and find missed notes. It works alongside Undocumented Visit Tracking.
Accessing the Report
In the left sidebar, go to Reports → Note Completion. Reports are listed directly in the main sidebar.
Filters
Previous period / Next period arrows: move backward or forward in time
Date range: Yesterday, Last 7 days, Last 30 days, Last 90 days, or Custom range
A custom range can be up to 1 year, for example January 1 through December 31.
The report refreshes when you change a filter.
How Notes Are Counted
Appointments: every completed appointment in the period.
Signed notes: completed appointments that have a signed Text Note or SOAP Form from that provider.
Missing notes: completed appointments with no signed note. These follow the same rules as Undocumented Visit Tracking:
Session types the provider ignores are left out.
Only visits after the provider's Start Tracking From date count.
Visits more than 90 days ago are not counted as missing, even if Start Tracking From is earlier.
Completion rate: signed notes divided by signed notes plus missing notes.
A provider with no Start Tracking From date has no missing notes and no completion rate. Their row shows No Start Tracking From date and a dash for those columns. To set the date, see Undocumented Visit Tracking.
Summary
At the top of the report you'll see Completion rate, Appointments, Signed notes, and Missing notes for the period.
By Provider
Each provider is listed with Appointments, Signed notes, Missing notes, and Completion rate. Select a column heading to sort.
Under each provider's name you'll see their Start Tracking From date. You can also:
Select View visits to see that provider's appointments that still need a note.
Select the info icon to see which session types the provider ignores.
If there are no completed appointments in the period, you'll see No appointments found.
Export CSV
Select Export CSV in the top right corner to download the report data.
