Manually Add Intake Form to Patient Profile

You can manually assign an intake form when it is not automatically triggered by send rules.


How to Manually Add a Form

  1. Open the client’s profile.

  2. Click the Forms tab.

  3. Click Create New Form.

  4. Select one or more intake form templates from the dropdown.

  5. (Optional) Choose an appointment to attach the form to.

  6. Click Create Forms.

The form will immediately appear as Pending in the client’s file.
